Based on that map it looks like we have three primary mainland offensive lauch sites ...or more like regions in ND, Mn and CO/WY/NE

It's a completely contrived map. Targeting errors aside, the only thing you need to know is that Soviet collective strike planning wasn't done in terms of "500 missiles, 2000 missiles".
